Norfolk and Suffolk NHS Foundation Trust provides child and adult mental health services, learning disability, wellbeing, older people's and eating disorder services across Norfolk and Suffolk.

Senior community Mental Health Nurse - Older People's Service

An exciting opportunity has arisen for a full-time Senior Community Mental Health Nurse to join the Older People's Community team in Ipswich.

We are seeking for a candidate who is highly innovative, enthusiastic and, passionate about working with older people and their carers with a person-centred and holistic approach. You will be part of psychologically minded and highly performing multidisciplinary team of clinicians. You will be delivering biopsychosocial interventions under the Recovery Model. You will work with older adults with dementia and functional conditions.


Main duties of the job

  • You will be responsible for a caseload of service users with organic or functional illnesses with physical health complexity
  • You will work as a member of the MDT ensuring that high quality individualised care is delivered and that this promotes recovery and maximises independence.
  • You will be responsible for carrying out full and thorough clinical and risk assessments for the people in your care, seeing them holistically, being sensitive to their needs and escalating risks to the MDT.
  • You will be responsible for completing Dialog+ care plans in collaboration with the service users and their carers when possible.
  • You will be responsible for sharing your expertise and knowledge with other team members to promote learning from each other and developing each others expertise.
  • You will attend monthly 1:1 supervisions and clinical group supervisions. You will participate in monthly protected learning days and quality improvement projects to ensure you contribute to and maintain positive change within your own work environment.

You will be part of a highly performing multidisciplinary team of clinicians with strongly embedded values of working together. Our team consists of psychiatrists, a clinical psychologist, a mental health and wellbeing trainee, mental health nurses, social worker, assistant practitioner, peer support worker and support worker. We are diverse in background, profession, and experience. In November the team achieved national accreditation with the Royal College of Psychiatrists in recognition of the high quality of care we provides for service users and their carers.

In the summer the team received a commendation for ‘Improving Quality Through Innovation’ as a finalist in the Trust Staff Excellence Awards.

We promote hybrid working. Professional meetings, telephone contacts and administration work can be completed remotely but we require office attendance to cover duty work and face to face appointments for the service users who require it.
