If you are an enthusiastic and forward-thinking Nurse (RMN), we are looking for you to join the Mental Health Triage and Assessment Team, based in Hull and East Riding of Yorkshire. We are seeking the right individual who can strive to deliver high-quality clinical interventions, and wants to further advance the way we provide care to some of the most vulnerable people in society.

Our Mental Health Triage and Assessment Team has recently moved into the planned care division and with this change we have improved our clinical pathways and brought closer integration between the assessment team and our community mental health services. Alongside the ongoing work of the community mental health transformation, this makes it an exciting time to join the team as the Clinical Lead.

As the Clinical Lead, you will work alongside the Team Manager, under the supervision of the Service Manager and Senior Clinical Lead. The team consists of registered clinicians and administrative staff, whom you will provide day to day support to.

You will work closely with key stakeholders, both internally and externally, to develop the service and support the approach of ‘No Wrong Door’. Additionally, you will commit to key drivers of ‘Trauma Informed Care’ and reducing the need for multiple assessments. You will work closely with your peers across the community mental health provision, to identify challenges and ways to address them. You will be responsible for development of clinical pathways and ensuring high quality triage and assessment is being undertaken through the use of appropriate audit and review of patient feedback.

Additionally, this role will continue to have a large direct clinical component, and you will be expected to carry out holistic assessments as part of your clinical role. You will be able to work flexibly and have the opportunity to work from home as a percentage of your time but will be required to work from site at other times.
