We are looking for an experienced a qualified social worker within the Churchill Hospital Social work team, based at the Churchill hospital in Oxfordshire.

Payrate: £35-38 per hour

Role Information:

• With a small number of acute medical wards, specialist wards and a mental health unit on site and providing support to out of county hospitals, the team plays a key role in the overall Health & Social Care provision to the people of Oxfordshire.
• Social Workers are vital members of the MDT, helping embed a strength-based approach to discharge planning and working together to support patient flow throughout the system.
• The pace is fast, the work is interesting and varied and this post is an opportunity to enhance your experience, skills, and knowledge at the interface of Health and Social Care.
• The contractual base for this role is the Churchill Hospital, however applicants will need to have the willingness and ability to travel in accordance with the de-mands of the work and the job profile
• This post is based in Adult Social Care and the post holder will be required to work collaboratively across adult social care and partner agencies to support and promote strong communities, making a real difference to the people and communities we serve, now, and for the future, whilst ensuring that we are de-livering value for money. Support is delivered through a strength-based ap-proach to create opportunities, build resilience and long-term support networks so that people live their lives as successfully, independently, and as safely as possible.
