An opportunity has arisen for enthusiastic and dynamic nurses to join our forward thinking, friendly and supportive community nursing team, at Stratton.
We are looking for nurses who possess strong communication skills and have the ability to be adaptable, reliable and approachable.
Whilst community experience would be an advantage, we are keen to recruit individuals who have relevant transferable skills and who are keen to develop their career in the community setting.
As a Community Nurse no day is the same and you will be met with patients of varied needs, from wound care to end of life care. This giving you the opportunity to develop within a supportive environment with opportunities for further career development.
Your role will be pivotal within the local community by making a difference to our patients whilst delivering a high level of care in their home environment.

As part of our team, you will be responsible for the assessment of nursing care needs, implementation and evaluation of care plans and the delivery of high quality, holistic care within the community setting.
You will need to be able to work independently, possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas if required to meet the needs of the service area. Travel is an essential part of this role, and you must be able to fulfil this requirement.
You will be required to support and wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life, including promoting self-care for patients, carers and families.
We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
