Job Description & Requirements Title: Physical Therapist – Home Health
Location: Ketchikan, Alaska
Duration: 26 weeks
Shift: Day shift, 4x10-hour shifts (07:00 AM – 05:00 PM); 5x8-hour schedule may also be available; no weekends or on-call required
Overview & Responsibilities:
Provide home health physical therapy services to adult and geriatric patients in a community-based setting.
\tPerform comprehensive patient evaluations, develop individualized plans of care, and establish short- and long-term functional goals.
\tDeliver skilled physical therapy treatments including therapeutic exercise, gait training, neuromuscular re-education, manual therapy, vestibular interventions, and patient/family education.
\tComplete OASIS assessments, reassessments, discharges, and all required documentation in compliance with regulatory and professional standards.
\tManage a caseload averaging 4 visits per day with an average daily census of 26 patients.
\tTreat common diagnoses including COPD, CHF, orthopedic conditions (including joint replacements), and neurologic diseases.
\tMonitor vital signs, cardiopulmonary status, pain levels, strength, sensory response, and safety risks in the home environment.
\tIdentify environmental barriers and implement safety and injury prevention strategies.
\tCoordinate care with interdisciplinary team members including social services and interpretation services.
\tProvide oversight or supervision to therapy assistants, aides, or students as applicable.
\tMay float to hospice services within scope if needed.
\tRequires reliable personal transportation for home visits.
\tRequired Experience:
Minimum of 1 year of physical therapy experience.
\tAt least 1 year of Home Health or OASIS Assessment (OA) experience required.
\tOASIS experience required for first-time travelers.
\tEpic EMR experience preferred (not required).
\tFirst-time travelers accepted with OASIS experience.
\tLicensure & Certifications:
Active Alaska Physical Therapist license required (compact license accepted).
\tBLS certification required.
